Submit a request
We will need:
- A Letter of Authorization completed. (this document is attached)
- A copy of your most recent phone bill.
You are able to attach the necessary documents at the bottom of the submit a request form.
Once you send this information, the process takes about a week.
When the port is complete, we charge your account $25 per number.
